To request an absentee ballot, fax or email this form (http://gab.wi.gov/forms/gab-121-english)

to the Sun Prairie City Clerk, Diane Hermann Brown at FAX 608-825-6879 or dhermann@cityofsunprairie.com. 

Absentee requests must be received in the Clerk's office by 5:00pm the Thursday prior to each election, in order for them to be mailed out.

You can vote by absentee ballot in person at City Hall Mon-Fri for the two weeks prior to the election (from March 30 to April 3).

Or you can vote in person on election at your regular polling place. Polls will be open from 7am to 8pm.
